Select the "Type" tool. Draw the text frame that will contain your form.
Enter the text for your first field.
Click on "Type" and select "Tab." Click on the "Right-Justified" icon. Click inside the "Leader" box and type "_" to create a line for the field. Click on the bar above the Tabs ruler where you want the line to end.
Position the cursor at the end of the first field text. Press the "Tab" key to create the line. Repeat these steps to create additional fields with underscore line.
Enter text for radial buttons.
Select the "Ellipse" tool or the "Rectangle" tool. Hold down the "Shift" key and draw a circle or square the size of the desired radial button or check box outside of the text frame.
Press "Control" and "X" to cut the circle. Position the text inside the text frame where you want the radial button to appear. Then, press "Control" and "V" to paste the button into the form. The button will reposition with the text if you make any changes.
Add any additional elements, such as instructions or images, to the form.
Click on the "File" menu and select "Export."
Name the file and select the desired location. Select "Adobe PDF (Interactive) from the "Format" drop-down menu. Then, press "Save."
